Non syndromic microphtalmia in 3 siblings

Objectives: We aimed to describe the clinical features of nonsyndromic microphthalmia through three cases in three siblings including female twins.

Introduction: Microphthalmia is a rare congenital or developmental anomaly in which the globe is abnormally small. Between one-third and one-half of affected individuals have microphthalmia as part of a syndrome that affects other organs and tissues in the body. These forms of the condition are described as syndromic. When microphthalmia occurs by itself, it is described as nonsyndromic or isolated.

Materials / method: Retrospective, descriptive study including three siblings with isolated microphthalmia. All three patients underwent complete ophthalmological examination, A-scan ultrasound biometry, B-scan ultrasonography and Optical Coherence Tomography. MRI examination has not been performed.

Results: We included three cases of nonsyndromic bilateral microphthalmia. All three patients were siblings: female twins and a male. Medical history taking did not note any environmental factors that might affect early development, such as nutrition shortage during pregnancy, radiation, infections (rubella) or exposure to teratogens. Mean best corrected visual acuity was 20/50 and all six eyes were amblyopic. Mean refractive error was +11.50 Diopters and axial length average was 17.2 mm. Cataract was observed in 3 eyes and microcornea in 2 eyes of the same patient, without any coloboma.

Conclusion: The management of microphthalmia appears to be challenging. Therapy aims to maximise existing vision and enhance cosmetic appearances rather than improve sight. Genetic counselling and antenatal diagnosis are highly recommended.

Isolated congenital alacrymia: about a case

Objectives: Le but de notre travail est de rapporter un cas d’alacrymie congénitale isolée et de décrire le tableau clinique, la démarche diagnostique ainsi que la prise en charge thérapeutique.

Introduction: L’alacrymie congénitale isolée est un trouble pédiatrique rare définie par l’absence ou l’hyposécrétion des larmes secondaire à l’aplasie ou l’hypoplasie des glandes lacrymales sans autres signes systémiques associés. Ce trouble est à l’origine d’un syndrome sec oculaire avec altération de la qualité de vie des patients et des complications ophtalmologiques redoutables imposant une prise en charge précoce.

Materials / method: cas clinique

Results: Il s’agit d’une fille de 3 ans, qui a consulté pour l’absence des larmes associée à un syndrome irritatif oculaire. L’acuité visuelle a été à 7/10 aux 2 yeux. L’examen a objectivé une kératite ponctuée superficielle diffuse accentuée en inférieur, le BUT a été inférieur à 1 seconde. Test de Schirmer 1 a été nul. Le reste de l’examen n’a trouvé ni xérostomie ni dysphagie ni retard staturo-pondéral ou mental. Le diagnostic d’alacrymie congénitale isolée a été porté. La patiente a bénéficié d’un traitement lacrymal substitutif à instillation fréquente et régulière avec bonne évolution.

Conclusion: L’alacrymie présente une circonstance de découverte des plusieurs syndromes polymalformatifs qui doivent être recherchés et éliminés en première intention. Le diagnostic reste un diagnostic d’exclusion vu la gravité et la variabilité des tableaux cliniques d’autres causes du syndrome sec chez l’enfant. Malgré qu’il soit rare le diagnostic d’alacrymie congénitale isolée doit être considéré lors du syndrome sec dans la population pédiatrique. L’imagerie a un rôle important dans le diagnostic positif d’aplasie et hypoplasie des glandes lacrymales.

The brakes to the good progress of a visual field

Objectives: Le but de notre travail est de rapporter les différents freins et difficultés à la réalisation d'un champ visuel réussi et leur impact sur l'organisation du travail au sein de l'unité d'orthoptie

Introduction: Le champ visuel (CV) est la partie de l'espace qu'un œil peut percevoir autour du point qu'il fixe. L'examen du CV a pour but de cartographier cet espace, quand l'œil fixe un point immobile situé droit devant lui. Il donne des renseignements irremplaçables. Malheureusement il s'agit d'un examen un peu fastidieux.

Materials / method: Un total de 890 CV réalisés du Janvier au Décembre 2018 au service d'ophtalmologie de l'hôpital des FSI la Marsa ont été analysés sur le plan: déroulement de l'examen, nombre de CV réalisés par patient et par consultation pour avoir un relevé réussi et interprétable, raisons qui peuvent expliquer la difficulté de réalisation de l'examen.

Results: Deux cents vingt patients ont réussi leur relevé du CV dès le premier examen, 350 dès le deuxième, 200 patients ont nécessité 3 essais et 120 patients n'ont pas pu obtenir un résultat interprétable après 3 relevés, ce qui fait 1880 CV fait pour 890 patients. Les difficultés étaient en rapport avec des causes intrinsèques (les âges extrêmes, la basse vision, une pathologie oculaire ou générale associée surtout la surdité, la maladie de Parkinson, les troubles psychiatriques, une personnalité de type A, la claustrophobie), la fatigue (manque de sommeil, long trajet...) le bruit...

Conclusion: La connaissance des freins à la réalisation d'un champ visuel est cruciale afin de trouver des solutions pour agir sur les facteurs modifiables. Une bonne organisation permettra de gagner du temps pour réussir une exploration qui n’est pas facile à réaliser et qui demande une attention importante de la part du patient.

Eye signs in Wernicke's Encephalopathy: about a case

Objectives: Objectifs :
Le but de notre travail est de rapporter un cas d’encéphalopathie de Wernicke révélée par des manifestations ophtalmologiques.

Introduction :
L'encéphalopathie de Gayet-Wernicke est une urgence diagnostique et thérapeutique résultant d’un déficit en thiamine (vitamine B1) . dans la forme typique, elle réalise une triade associant un syndrome confusionnel, des troubles oculomoteurs avec nystagmus et un syndrome cérébelleux (ataxie). Classiquement associée à l’alcoolisme chronique, elle peut toutefois être observée dans le cadre d'un hyperemesis gravidarum.
Matériels et methodes :
Cas clinique
Observation :
Patiente de 24 ans, enceinte à 15 SA, présentant des vomissements incoercibles avec pancréatite aiguë de stade B et à j4, une baisse brutale profonde de la vision bilatérale. L’examen a retrouvé une AV à voit la main bouger à l’OD et à la perception lumineuse à l’OG et un œdème papillaire de stade 2 bilatéral. L’IRM cérébrale a mis en évidence des hypersignaux FLAIR bithalamiques postéro-internes et symétriques, péri-aqueducal et des corps mamillaires évoquant une encéphalopathie de Gayet Wernique. Secondairement, le tableau s’est complété par l’apparition d’une marche ataxique. Une supplémentation intraveineuse en vitamine a été instaurée puis par voie orale avec évolution très favorable.
Conclusions :
Le syndrome de Gayet-Wernicke est une pathologie grave, pouvant en jeu le pronostic vital materno-fœtal Bien que les manifestations ophtalmologiques les plus fréquentes soient la diplopie et le nystagmus, la présence d’autres atteintes comme l’œdème papillaire bilatéral chez notre patiente doit faire évoquer le diagnostic et imposer la mise en route d’un traitement.

Postoperative macular edema (Irvine-Gass syndrome): clinical and therapeutic aspects

Objectives: Objectifs :
Le but de notre travail est de préciser les particularités cliniques et l’approche thérapeutique de l’œdème maculaire postopératoire à travers trois observations.

Introduction :
Le syndrome d’Irvine-Gass, décrit pour la première fois en 1953, consiste en un œdème maculaire cystoïde (OMC) qui se manifeste dans les suites d’une chirurgie de la cataracte.
Observation :
Il s’agit de trois patients, sans antécédents pathologiques particuliers qui ont présenté dans les suites d’une chirurgie de la cataracte non compliquée (réalisée par phacoémulcification) une récupération fonctionnelle imparfaite avec un examen à la lampe à fente normal à part une hyperhémie papillaire. L’OCT-SD a permis d’objectiver la présence d’un OMC. L’angiographie à la fluorescéine a montré outre l’OMC une hyperfluorescence papillaire au temps tardif. Un traitement comprenant des anti-inflammatoires stéroïdiens(AIS) et non stéroïdiens (AINS) par voie topique et acétazolamide par voie orale a été prescrit avec une bonne évolution clinique et tomographique.
Discussion et conclusions :

Le syndrome d’Irvine-Gass apparaît en général entre 1 et 3 mois après la chirurgie de la cataracte et son incidence varie de 3 à 60 % selon les études.
Même si sa physiopathologie est encore soumise à discussion, il semblerait qu’une augmentation des médiateurs inflammatoires soit à l’origine d’une hyperperméabilité de la barrière hémato-rétinienne.
Les facteurs de risques sont le diabète, la rétinopathie diabétique avec un risque augmentant avec la sévérité de celle-ci, l’uvéite intermédiaire ou postérieure et les complications peropératoires.
L’OCT-SD permet d’objectiver la présence de l’œdème maculaire et de surveiller sa résorption ou sa réapparition à l’arrêt ou à la décroissance du traitement. L’altération de la ligne de jonction IS/OS des photorécepteurs est significativement associée à une mauvaise récupération visuelle.
L’évolution spontanée des OM postopératoires est dans la majorité des cas favorable. Sa prévention repose sur les collyres AIS et AINS.
Concernant le traitement, l’administration topique de collyres AINS et AIS reste l’attitude privilégiée en première intention. L’adjonction d’un inhibiteur de l’anhydrase carbonique peut être associée per os en cas d’échec de la thérapeutique locale.
En cas d’œdème réfractaire, d’autres thérapeutiques peuvent être tentées.

Hyperplasia and persistence of primary vitreous: about 2 cases

Objectives: 1: Reconnaître une hyperplasie et persistance du vitré primitif devant une leucocorie et une présence d’une vascularisation rétrolentale pouvant orienter vers une cataracte et un rétinoblastome.
2: Connaitre qu'une membrane pupillaire est un signe associé à la persistance du vitré primitif
3: Connaître les différentes modalités thérapeutiques

Introduction: L’hyperplasie et persistance du vitré primitif (HPVP) est une malformation oculaire congénitale liée à un échec de régression du système hyaloïdien fœtal et du vitré primitif hyperplasique à la naissance. L’association à une membrane pupillaire se voit dans 3% des cas. Nous rapportons à ce propos 2 cas de PVP.

Materials / method: Nous rapportons l’observation de 2 enfants âgés respectivement de 5 ans et 14 ans, consultant pour une leucocorie et ésotropie unilatérale. On a réalisé un interrogatoire et un examen ophtalmologique complet. Un bilan comportant une échographie doppler oculaire a été demandé.

Results: L’examen clinique des 2 enfants retrouve une AV basse, et une défense à l’occlusion. L’examen microscopique de l’enfant de 5 ans montre une cataracte blanche obturante, associé à des reliquats de membrane pupillaire pigmentés. L’examen de l’enfant de 14 ans trouve une microcornée, une cataracte blanche régressive, des SIC et une saillie des procès ciliaires. L’échographie oculaire des 2 patients révèle une persistante d'artère hyaloïde s'étendant du cristallin au disque optique. Nos 2 patients ont bénéficié d’une phacoaspiration et une vitrectomie antérieure.

Conclusion: La PVP constitue un défi pour les ophtalmologistes en raison de la difficulté d’obtenir un bon résultat visuel à la fin du traitement.

Thrombosis of the cavernous sinus complicating an orbital cellulitis: about a case

Objectives: Les objectifs de notre travail sont:
- de mettre en valeur l'importance d'une prise en charge précoce d'une cellulite rétroseptale
- étudier les caractéristiques cliniques et radiologique de la thrombose du sinus caverneux.

Introduction: La cellulite orbitaire rétroseptale est une pathologie rare qui constitue une urgence ophtalmologique. En effet l'évolution vers la thrombose du sinus caverneux est la complication redoutable des cellulites orbitaires. Nous rapportons à ce propos le cas d’une cellulite orbitaire compliquée d’une thrombose du sinus caverneux.

Materials / method: Nous rapportons l’observation d’un enfant âgé de 13ans, consultant pour un œdème palpébral douloureux de l’œil gauche associé à un proptosis et une baisse d’acuité visuelle. On a réalisé un interrogatoire et un examen ophtalmologique complet. Un bilan comportant une TDM orbito-cérébrale a été demandé.

Results: L’examen clinique de l’enfant retrouve une AV basse un œdème palpébrale chaud et douloureux de l’OG. L’examen ophtalmologique montre une exophtalmie associée à un chémosis et une limitation de l’oculomotricité (OM)dans le regard latéral. La TDM orbito-cérébrale révèle une cellulite rétroseptale compliquée d’une thrombose du sinus caverneux et une sinusite maxillaire et ethmoïdale. L’enfant a bénéficié d’une biantibiothérapie par voie parentérale associée à une corticothérapie générale et une héparinothérapie. L’évolution était marquée par l’amélioration de l’œdème palpebral et de l'OM.

Conclusion: La thrombose du sinus caverneux compliquant une cellulite orbitaire est une pathologie rare, mais grave. Le diagnostic repose sur des arguments cliniques et doit être confirmé par l’imagerie. Cependant toute cellulite orbitaire doit être reconnue précocement et traitée de façon énergique.

Congenital family aniridia: about 3 cases

Objectives: Le but de notre travail est d’étudier les différents aspects cliniques de l'aniridie congénitale et les différentes complications de la maladie ainsi que la difficulté de prise en charge à travers 3 cas cliniques.

Introduction: L’aniridie est une atteinte génétique rare, par mutation du gène Pax6. Elle est caractérisée par l’absence quasi totale de l’iris. Elle peut être isolée ou associée à une atteinte systémique. Nous rapportons à ce propos le cas de 03 patients, atteints d’aniridie congénitale

Materials / method: Nous rapportons l’observation de 3 cas, une mère et ses deux filles consultant pour une aniridie. On a réalisé un interrogatoire et un examen ophtalmologique complet. Un bilan comportant une échographie rénale et un OCT, a été demandé.

Results: La mère âgée de 40 ans présente une exotropie de l’OD et une AV réduite à CLD. L’examen microscopique note de manière bilatérale, une aniridie, une dystrophie de cornée avec pannus vasculaire et une cataracte congénitale bilatérale avec ectopie du cristallin. L’examen de sa fille de 18 ans trouve une AV réduite à 3/10 en ODG, une exotropie de l’OG et au niveau des 2 yeux, une dystrophie de cornée, une aniridie totale et une ectopie cristallinienne. L’examen de la fille de 18mois retrouve une aniridie totale et un nystagmus. Le fond d’oeil de cette famille retrouve une hypoplasie maculaire.

Conclusion: L’aniridie pose des problèmes de prise en charge. Une surveillance régulière et un conseil génétique demeurent essentiels pour pouvoir dépister les complications locales et générales.

Dermal limb revealing Goldenhar syndrome: about two cases

Objectives: un examen complet et attentif de tout enfant porteur d'une malformation est nécessaire, car susceptible de déceler un syndrome malformation parfois plus complexe .

Introduction: Le dermoïde du limbe en est la forme la plus courante de l'ensemble des tumeurs cornéo-conjonctivales à localisation limbique.
Elle peut s'associer à d'autres anomalies pour définir un syndrome du premier arc : le syndrome de Goldenhar, ou syndrome oculo-auriculo-vértébral qui représente le modèles des syndromes malformatifs en rapport avec les dysembryoplasies des arcs branchiaux. Le demoide du limbe est une lésion d’evolution benigne dont les seules conséquences possibles serait un astigmatisme dans le développement visuel

Materials / method: le premier cas une jeune de 19ans qui présente depuis la naissance une lésion tumoral conjontivale limbique avec des tumeurs dermoide du culs de sac conjonctivale en odg. L’examen des annexes retrouvait des dermolipomes palpébraux supérieur et inferieurs. Le second cas était un nourisson de deux ans, adressé dans le cadre du bilan général d’un syndrom polymalformatif.l ’examen ophtalmologique retrouvait une lésion tumorale conjonctivale limbique temporale inférieure, avec un fin duvet à sa surface evoaquant un dermoide du limbe.

Results: L'intervention menée sous anesthésie locale se déroula sans incident, se limitant à une simple résection de la partie en relief du dermoïde du limbe avec une bonne évolution clinique par la suite pour la première patiente.

Conclusion: Le dermoide du limbe est une lésion de diagnostic simple et d’évolution benigne . Cependant devant tout dermoide du limbe, il convient d’examiner l’aspect du massif facial, de rechercher minutieusement des lésions peri-auriculaires, vertebrales et costal pouvant s’inscrire dans le cadre du syndrome de goldenhar

Association between C21orf91 gene and HSV keratitis

Objectives: To describe single nucleotide polymorphisms (SNPs) of C21orf91 rs1062202 and rs10446073 in patients with herpetic keratitis by evaluating corneal sub-basal nerves, Langerhans cell (LC) and endothelium cell (EC) density during the acute phase of the disease.

Introduction: C21orf91 gene, also known as the Cold Sore Susceptibility Gene 1 is associated with herpes labialis manifestation [1,2]. This is the first study trying to find any possible associations between the Cold Sore Susceptibility Gene and herpetic eye disease.

Materials / method: A prospective clinical study included 260 subjects: 70 with herpetic eye disease, 101 with previous history of herpes labialis but no history of herpetic eye disease, and 89 with no history of any HSV diseases. All subjects underwent a complete ophthalmological examination including in vivo laser scanning confocal microscopy of the central cornea. C21orf91 rs1062202 and rs10446073 were genotyped using the real-time PCR method with Rotor-Gene Q real-time PCR quantification system. SNPs were determined using TaqMan genotyping assay, according to the manufacturer's manual.

Results: The C21orf91 rs10446073 genotype GT was more frequent in the HSV keratitis group, compared with healthy controls (20.0% vs. 7.9%), OR 2.929[1.11-7.716] (p<0.05). The rs10446073 genotype TT was more frequent in healthy controls (12.4% vs. 1.4%), OR 22.0[2.344-260.48] (p<0.05). The rs10446073 genotype GT increased the risk of EC density being less than 2551.5 cell/mm2, OR 2.852[1.248-6.515] (p<0.05). None of the SNPs and their genotypes influenced the LC density and corneal sub-basal nerve parameters, the type of herpetic keratitis or the presentation of herpes labialis (p>0.05).

Conclusion: Our study reports a new association between herpetic keratitis and human gene C21orf91: rs10446073 genotype GT being more common in herpetic keratitis patients and increasing the risk for the disease 2.9 times.

Serpiginous-like choroiditis: about a case and literature data

Objectives: Décrire les signes cliniques de la choroïdite serpigineuse-like et les examens complémentaires à demander en première intention avec intérêt particulier pour l'angiographie à la fluorésceine

Introduction: Parmi les manifestations intraoculaires de la tuberculose, la choroïdite serpigineuse like représente une forme clinique rare de la maladie , bien que cette association a été reconnue depuis des décennies. Nous rapportons une observation de choroïdite serpigineuse d'origine tuberculeuse.

Materials / method: Patient de 18 ans sans antécédents particuliers qui consulte pour BAV de l’OD d’installation rapidement progressive.
L’examen trouve une AV à 2/10 , un tyndall cellulaire (x) en chambre antérieure ,une hyalite (x) avec au FO des lésions choroïdiennes jaunâtres en carte géographique, irrégulières, à bords flous au pôle postérieur. L'examen de l'oeil gauche est sans particularité.
L’AF aux temps précoces montre une hypofluorescence suivie d'une hyper-fluorescence des bords des lésions aux temps tardifs
L’OCT montre une désorganisation des couches rétiniennes externes avec hypertrophie de l'EPR

Results: Les uvéites tuberculeuses peuvent se présenter sous la forme d'une choroïdite serpigineuse-like ou encore appelée choroïdite serpigineuse multifocale d'origine tuberculeuse.
Les deux yeux sont souvent touchés de manière asymétrique.
L'AF montre aux temps précoces une hypo fluorescence des lésions actives, suivie d'une hyper fluorescence tardive de façon centripète. Les lésions cicatricielles peuvent être hypo fluorescentes.
L'OCT objective la présence ou non d’œdème maculaire.
Le traitement est basé sur l'association antituberculeux et corticoïdes.

Conclusion: La choroïdite serpigineuse like est une manifestation rare de la tuberculose intraoculaire.Elle survient le plus souvent dans le cadre d'une tuberculose présumée ou latente dont la difficulté réside dans la mise en évidence de mycobactéries en intraoculaire. Le diagnostic doit être évoqué devant toute choroïdite serpigineuse-like au caractère angiographique très spécifique ,notamment en pays d'endémie tuberculeuse afin de proposer un traitement adapté.

Efficacy of treatment of palpebral pyogenic granuloma by topical timolol: about a case

Objectives: Les objectifs de notre travail sont:
- de rapporter l'efficacité du traitement des granulomes pyogéniques palpébraux par timolol topique.
- de mettre en valeur les avantages du traitement par timolol topique dans les granulomes pyogéniques.

Introduction: Le granulome pyogénique est une tumeur vasculaire bénigne qui survient le plus souvent suite à un traumatisme, à une brûlure, ou une chirurgie, mais qui peut également apparaître, -plus rarement- spontanément. Le traitement est variable, et peut consister en une exérèse chirurgicale, par laser, par électrocautérisation, mais également être moins invasif au moyen d’un traitement topique par bêta bloquants.

Materials / method: Une patiente de 38 ans s’est présentée en consultation pour une lésion de la région supéro-nasale de sa paupière supérieure droite, évoluant depuis 10 jours, rapportant une notion de saignement de la lésion 3 jours avant sa consultation.
L'examen clinique a permis d'évoquer le diagnostic de granulome pyogénique et de mettre en place un traitement non invasif par timolol topique 0,5% à raison de deux applications par jour.

Results: L’évolution a été marquée par une diminution progressive de la taille de la lésion, après 6 semaines de traitement, avec une disparition quasi totale de la lésion à l’issue de 11 semaines de traitement.
La patiente n’a rapporté aucun effet secondaire au traitement topique par timolol.
Le suivi de notre patiente à 4 mois n’a rapporté aucun signe de récidive locale du granulome pyogénique.

Conclusion: Plusieurs cas de granulomes pyogéniques traités par bêta-bloquants topiques sont décrits dans la littérature, chez l’adulte comme chez l’enfant, et rapportent des résultats très satisfaisants, avec une application facile ainsi qu’une bonne tolérance locale du produit.
Cependant, de plus larges études manquent à ce jour afin d’affirmer la réelle efficacité de ce traitement dans la prise en charge des granulomes pyogéniques.

Preoperative intravitreal dexamethasone implantation in patient for cataract surgery with chronic recurrent uveitis

Objectives: Preoperative intravitreal DEX implantation may be useful for preventing the aggravation or recurrence of uveitis after cataract surgery in patients with chronic recurrent uveitis.

Introduction: There are no studies have reported the effectiveness of preoperative DEX implantation in uveitis patients with not only quiescent inflammation but also active inflammation in cataract surgery to prevent the recurrence or aggravation of inflammatory activity. Therefore, we evaluated the effects of preoperative intravitreal DEX implantation on the control of inflammatory activity in patients with chronic recurrent uveitis who were to undergo cataract surgery.

Materials / method: Thirty eyes diagnosed with chronic recurrent noninfectious uveitis were included in the study. These eyes received an intravitreal implantation of 0.7 mg DEX (Ozurdex; Allergan, Inc, Irvine, California, USA) before cataract surgery. The visual outcome and the grade of uveitis were measured preoperatively and postoperatively.

Results: The best-corrected visual acuity was significantly higher at postoperative week 1 and months 1, 2, and 6 compared to baseline. The mean grade of anterior chamber cells was significantly lower at postoperative months 1, 2, and 6 compared to baseline. As the interval between DEX implantation and cataract surgery increased, so did the difference in the mean grade of AC cells after and before surgery. Results of log-rank tests significantly differed at 6 months between patients given systemic immunosuppressive agents and those not given such agents, but not between those with an active uveitis.

Conclusion: Preoperative intravitreal DEX implantation is useful for preventing the aggravation or recurrence of uveitis after cataract surgery in patients with chronic recurrent uveitis. Furthermore, a shorter time interval between implantation and surgery results in fewer AC cells postoperatively. We recommend using systemic immunosuppressive agents to prevent the recurrence of uveitis after this surgery.

Factors associated with corneal astigmatism change after ptosis surgery

Objectives: To evaluate possibility in prediction of postoperative cornea astigmatism change in ptosis patients who underwent ptosis surgical repair. Also, to compare the prevalence of corneal astigmatism pattern defined as with the rule (WTR), against the rule (ATR) and oblique astigmatism in preoperative ptosis patients and trend of axis change after surgery.

Introduction: Corneal astigmatism is a refractive problem commonly observed among ptosis patients. Explanation has been proposed by many studies believing that this was a result of eyelid pressure against cornea, leading to change of corneal shape. There are also several reports show that ptosis surgical repair can cause variation of change in corneal astigmatism both in diopter and axis, some show significant results. Few studies have been conducted to specify factors which are associated with degree of astigmatism change after ptosis surgery.

Materials / method: Forty two eyes of 28 patients , underwent levator resection, were retrospectively evaluated. Patient factors including age, sex, preoperative MRD1 and preoperative corneal astigmatism were classified into age >50 and <50, MRD1 -2-0 and 1-3, Preoperative corneal astigmatism <1.5 D. and >1.5 D, axis WTR ,ATR and oblique . Preoperative corneal astigmatism, 6 weeks postoperative corneal astigmatism and degree of postoperative astigmatism change between subgroups of each factors were analyzed as well as prevalence of pattern of axis (WTR, ATR and oblique ) preoperative and postoperative.

Results: Overall, mean postoperative astigmatism showed a reduction value compared to mean preoperative astigmatism in all subgroups, excepted for subgroup of patient with astigmatism <1.5 D. and patient with oblique astigmatism. Analysis of preoperative corneal astigmatism value compared to postoperative corneal astigmatism using dependent T-test showed significant value in only one subgroup which is patient with preoperative astigmatism >1.5 D. (p=0.0042). Majority of preoperative patients demonstrated WTR astigmatism pattern(45%) with reduction of degree of WTR astigmatism post operative.(68.42%)

Conclusion: This study demonstrates first time data suggesting that more severity of preoperative corneal astigmatism defined by more than -1.5 diopters is associated with significant change in postoperative corneal astigmatism in ptosis patients undergoing ptosis surgery, implicating the important of severity evaluation of corneal astigmatism before planning ptosis surgery as well as cataract surgery. Thus, we encourage considering of corneal astigmatism severity before planning cataract surgery in ptosis patient, especially with toric-IOL, to avoid possibility of error calculation.

Bilateral congenital sclerocornea associated with polymalformative syndrome

Objectives: Présenter un cas rare de sclérocornée congénitale bilatérale associée à un syndrome polymalformatif.

Introduction: La sclérocornée congénitale appartient au groupe des opacifications cornéennes congénitales. Elle se présente comme une lésion non évolutive, non inflammatoire, périphérique, centrale ou sectorielle. C’est une pathologie génétique qui apparait sporadiquement ou selon une transmission autosomique dominante.

Materials / method: Un enfant de 5 ans, consulte au service d’ophtalmologie pour une cécité́ bilatérale évoluant depuis la naissance. Il est le deuxième d’une fratrie de deux enfants, né d’un mariage non consanguin, déroulement de la grossesse et accouchement normaux. Dans ses antécédents, sa maman est porteuse d’une sclérocornée congénitale unilatérale.

Results: Son acuité visuelle se résumait à une perception lumineuse bilatérale. L’examen montrait une opacification totale des deux cornées, plus marquée à gauche avec une néovascularisation cornéeene (Fig.1 et 2). Les deux chambres antérieures étaient étroites (Fig. 3). L’échographie oculaire montrait un segment postérieur sans particularités et une longueur axiale normale au niveau des deux yeux. Un examen pédiatrique à la recherche d’autres malformations associées avait été réalisé, montrant une une tétralogie de Fallot, des lésions du vélopharynx, un déficit en hormone de croissance et une scoliose

Conclusion: La sclérocornée congénitale est une pathologie rare. La recherche des anomalies du clivage de la chambre antérieure est obligatoire avant toute prise en charge. La kératoplastie transfixiante est le traitement de choix.

Orbital plasmacytoma in Kahler's disease

Objectives: A travers cette observation, les auteurs rapportent le cas d’un plasmocytome orbitaire chez une patiente suivie pour myélome multiple.

Introduction: Le plasmocytome orbitaire est une localisation rare de la maladie de Kahler. Il peut apparaître comme forme solitaire, dans le cadre du plasmocytome extramédullaire ou encore, lors de l’évolution d’un myélome multiple connu.
Les manifestations cliniques sont non spécifiques, et le diagnostic est essentiellement histologique.
Le traitement n’est pas bien codifié, il fait appel à l’association radiothérapie externe chimiothérapie et rarement la chirurgie.
A travers cette observation, les auteurs rapportent le cas d’un plasmocytome orbitaire chez une patiente suivie pour myélome multiple

Materials / method: Patiente âgée de 64 ans suivie pour maladie de Kahler.
L’examen clinique retrouve un œdème palpébral ecchymotique, une exophtalmie axile douloureuse à la palpation, non réductible avec bourgeon tumoral au niveau de la région caronculaire, Limitation minime des mouvements oculaires ainsi qu’un chémosis conjonctival hémorragique L’acuité visuelle est à perception lumineuse et le RPM est absent. Le fond d’œil retrouve un œdème papillaire diffus.

Results: ->TDM Crânio-cérébrale (Fig. 2): Processus lésionnel de l’orbite gauche rehaussé par le produit de contraste, infiltrant la graisse intra et extra conique, le muscle droit interne et les gaines du nerf optique.
->BIOPSIES
Orbitaire (Fig. 3): Plasmocytome orbitaire.
Cutanée (masse scapulaire) : localisation cutanée d’une prolifération plasmocytaire à chaînes kappa.
->TRAITEMENT ET ÉVOLUTION
Radiothérapie externe (30 Gy)
Evolution (recul de 3 mois) : (Fig. 4) Amelioration de l’acuité visuelle ( mouvement des doigts), régression modérée de l’exophtalmie

Conclusion: Les auteurs relatent, à travers ce cas, une localisation orbitaire ; qui reste rare ; de la maladie de Kahler, ainsi que la place de l’histologie dans le diagnostic positif.

Retinal myelin fibers: about 3 cases

Objectives: Préciser les particularités cliniques et diagnostiques des fibres à myéline rétiniennes.

Introduction: Les fibres à myéline sont des anomalies congénitales des fibres rétiniennes peu fréquentes mais de diagnostic aisé malgré une pathogénie encore incertaine. Ces lésions le plus souvent asymptomatiques peuvent cependant être associées à d’autres atteintes oculaires et notamment une myopie qu’il faudra rechercher.

Materials / method: Observation 1 et 2:
L ‘examen de la jeune fille agé de 12ans retrouve une acuité viuelle à 10/10e après correction (-1,5 D Sph ODG). L’examen du segment antérieur est normal. Le fond d’oeil retrouve à gauche, un placard blanchâtre papillaire et péri papillaire, plan à bord festonné masquant les vaisseaux (photo1) évoquant le diagnostic de fibres à myéline. Le champ visuel de Goldman a été réalisé montrant un élargissement de la tache aveugle (photo2)
L’examen ophtalmologique des membres de la famille a retrouvé le même aspect chez la mère (photo 3).

Results: Observation 3:
Il s’agit d’une fille de 15 ans chez qui l’examen ophtalmologique note à l’œil droit une acuité visuelle à 10/10e après correction (-2 D Sph) avec un segment antérieur et FO sans particularités et à l’œil gauche une acuité visuelle à CLD à 2m non améliorable, la réfraction ayant montré une myopie de -10 D. Le fond d’œil montre un aspect de fibres à myéline péripapillaires étendues et une atrophie choriorétinienne modérée.(photo4) L'échographie en mode A retrouve une longueur axiale de 27 mm.
La patiente a bénéficié d’une cure d’amblyopie et l'acuité visuelle finale est 2/10.

Conclusion: Les fibres à myéline sont des lésions bénignes et souvent asymptomatiques cependant elles peuvent être associées à d’autres atteintes oculaires notamment une myopie pouvant être a l’origine d’une amblyopie de mauvais Pronostic d’où l’importance du dépistage d’une anomalie réfractive en cas de fibres à myéline unilatérales, et d’une thérapie occlusive assez agressive.

Decompensated central retinal vein occlusion

Objectives: The aim of reporting this case is to elaborate on the importance of correcting the risk factors before deciding treatment options especially in non ischaemic CRVO .

Introduction: Central retinal vein occlusion (CRVO) is a relatively common retinal vascular disorder in which macular oedema may develop, with a consequent reduction in visual acuity, The pathogenesis of CRVO is believed to follow the principles of Virchow’s triad for thrombogenesis, involving vessel damage, stasis and hypercoagulability. The central retinal vein and artery share a common adventitial sheath at arterovenous crossings posterior to the lamina cribrosa so that atherosclerotic changes of the artery may compress the vein and precipitate CRVO[. Materials / method: Thirty years old female presented to the casuality clinic as areferral from the maternity ward with four days duration of blurry vision in her right eye since she gave birth , on examination , visual acuity 6/5 right eye , 6/9 left eye , pupil reactive and rounded with no RAPD .
Intra ocular pressure 15/15 both eyes , clear cornea , quiet and deep anterior chamber .
Dilated fundus examination with 1% tropicamide and 2.5% phenylephrine showed left eye central retinal vein occlusion with dos and blots haemorrhage in four quadrants and no signs of any ischaemia , no cotton wool spots .

An audit on performance of fundoscopy in patients presenting with headache to the acute medical take at Luton and Dunstable University Hospital

Objectives: - Be familiar with the British Association for the Study of Headache (BASH) National Headache Management System for Adults 2019 guidelines
- Learn about the compliance of the acute medical department at Luton and Dunstable University Hospital with BASH guidelines on fundoscopy
- Learn about the results of a survey that was carried out amongst doctors within Luton and Dunstable University Hospital. The survey gathered information on current awareness of BASH guidelines, how often doctors currently undertake fundoscopy in patients presenting with headache, what prevents doctors from carrying out fundoscopy in these situations, and what interventions they felt would increase the likelihood of carrying out fundoscopy

Introduction: Headache is a common presenting complaint to the acute medical take. BASH (British Association for the Study of Headache) National Headache Management System for Adults 2019 guidelines recommend an appropriate neurological investigation including fundoscopy when assessing a patient with headache. Luton & Dunstable is a district general hospital serving a population of approximately 300,000. We sought to assess our compliance as an acute medical department with BASH guidelines on fundoscopy and how to improve it.

Materials / method: A retrospective review of all medical clerkings where the presenting complaint was headache from 01.08.17 – 30.09.17 was carried out. These were audited against 2019 BASH guidelines. Data was collected on patient demographics and details of the physical examination performed.
A survey was emailed to doctors within the trust to gather information on awareness of BASH guidelines, what prevented them from carrying out fundoscopy and what interventions they felt would increase the likelihood of them doing so.

Results: 59 medical clerkings for headache were assessed with ages ranging from 18 to 70+ years; 38 patients (64%) female and 21 (36%) male. 52 patients (88%) did not have fundoscopy performed as part of their initial clerking, and none had it done on the post take ward round.

Of the 33 survey responses, 9% of respondents always performed fundoscopy; 42% did sometimes, and 49% never. 70% were not aware of BASH guidelines. The most common reasons as to why fundoscopy was not performed were lack of equipment/ awareness of location, lack of confidence in technique and ability to interpret findings.

Conclusion: The majority of patients presenting with headache to the acute medical take at Luton & Dunstable Hospital do not have fundoscopy performed as recommended by BASH guidelines. Aside from a majority lack of awareness of these guidelines, the main obstacles to performing fundoscopy are a lack of readily available ophthalmoscopes and dilating eye drops as well as low confidence in technique and interpretation of results. We hope to introduce measures to improve this and re-audit in due course.

Bilateral pellucid marginal degeneration: about a case

Objectives: Le but de notre travail est d’étudier les caractéristiques cliniques et topographiques de la maladie, son diagnostic différentiel et les stratégies thérapeutiques à travers une observation et une revue de la littérature.

Introduction: La dégénérescence marginale pellucide est une maladie non inflammatoire progressive ectasiante,affectant la partie inférieure de la cornée. Elle est souvent confondue avec le kératocône pour des similitudes cliniques, histologique et topographiques.

Materials / method: Nous rapportons le cas d’une patiente âgée de 48ans, opérée dune cataracte post-traumatique il y a 12 ans de œil droit, et ayant une soeur ayant bénéficié d’une KT ; qui se présente en ophtalmologie pour baisse progressive de l’acuité visuelle de l’œil droit depuis 6mois. L’examen clinique trouve une acuité visuelle réduite a MDD de OD et 2/10 en OG. En biomicroscopie, nous avons observé une bande de cornée amincie en inférieur, et séparée du limbe par un croissant de cornée normale, dont l’aspect est beaucoup plus accentué au niveau de OD.

Results: La topographie de l’œil droit montre un amincissement cornéen qui prédomine en position inférieure sur la carte pachymétrique. Les cartes d’élévation objectivent une saillie cornéenne au-dessus de la zone d’amincissement. Sur la carte axiale, on note un aplatissement des hémi méridiens verticaux supérieurs Un astigmatisme antérieur de 17 D à 87° relevé pour de l’OD, et de 10 D à 93° pour l’OG. En guise de traitement, un cross-linking de OD a été réalisé , et la pose d’une lentille sclérale a été proposée.

Conclusion: La DMP est une affection idiopathique, rarement unilatérale et responsable d’un astigmatisme irrégulier. La topographie cornéenne tient une place de choix dans son diagnostic. Plusieurs interventions chirurgicales ont été utilisées pour tenter d'améliorer l'acuité visuelle lorsque les lunettes et les lentilles de contact ne permettent pas une correction visuelle adéquate.

Chronic blepharitis revealing discoid lupus erythematosus (SLE): about a case

Objectives: Démontrer l’intérêt du diagnostic précoce, clinique et histopathologie, de l’atteinte palpébrale isolée dans le LED et son importance dans la prévention des complications locales.

Introduction: Le lupus érythémateux discoïde (LED) est la forme la plus courante de lupus érythémateux cutané chronique (LECC). Les lésions discoïdes se présentent sous forme de plaques érythémateuses bien définies présentant une écaille adhérente épaisse et un colmatage folliculaire. Dans les lésions anciennes, on peut observer des cicatrices atrophiques accompagnées d’hyperkératose verruqueuse. Le LED affecte généralement la peau photo-exposée, le plus souvent le visage. Une atteinte des paupières inférieures survient chez 6% des patients; toutefois, les lésions des paupières sont rarement isolées.

Materials / method: Nous rapportons le cas d’un homme de 45 ans, soudeur de profession, qui s'est présenté en janvier 2018 pour une rougeur de la paupière inférieure gauche persistante depuis deux ans. Il avait été traité pour une blépharite et un eczéma par des antibiotiques topiques et des corticostéroïdes. L'examen dermatologique a révélé une plaque érythémato-atrophique sur le tiers interne de la paupière inférieure droite. Les structures internes de l'œil n'ont pas été touchées.

Results: Les examens complémentaires ont montré un AAN positif et un Anti DNA négatif. L'analyse de la biopsie a objectivée des lésions typiques de LED: hyperkératose folliculaire, dégénérescence vacuolaire et infiltrat focal lymphocytaire dans le derme. Le traitement comprenait 250 mg de chloroquine par jour, une pommade d’hydrocortisone à 1% et une photoprotection. La lésion discoïde sur la paupière s'est complètement résorbée après quatre semaines. Le traitement à la chloroquine a été poursuivi pendant huit semaines. Le patient fut examiné tous les trois mois et aucune lésion n’est apparu depuis.

Conclusion: Le LED palpébral doit être différencié du dysfonctionnement des glandes de Meibomius, du carcinome sébacé et basocellulaire, du lymphome, du psoriasis, de la dermatite séborrhéique, de la rosacée, de la sarcoïdose et de la dermatite de contact. Le diagnostic et le traitement précoces de la maladie, sont importants pour éviter un dysfonctionnement palpébral, qui peut être à l’origine de cicatrices, de synéchies, de cils trichiasis, d'entropion et d'ectropion. Enfin, un suivi au long cours est primordial, afin d'identifier les récidives et de réinstaurer un traitement si nécessaire.

Huge excavated optic disk coloboma with restriction of eye movements

Objectives: to report a case of huge excavated optic disk coloboma which also caused restriction of eye movements.

Introduction: The initial steps of human eye arrangement begin with the formation of two optic vesicles originating from the neural tube. Anterior and posterior walls of each of these vesicles then approach together and make the two-layered optic cup which is incomplete and has a choroidal fissure. Deficient arrangement and closure of this fissure during embryonic development may result in the absence of normal eye tissues including the cornea, iris, choroid, retina, and the optic nerve; depending on its size and location. Materials / method: A 4-year-old girl was referred by an optometrist to our clinic due to unexplained reduced right vision in routine screening. She had no history of premature birth, prenatal or postnatal problems. 48 hours home intraocular pressure and blood pressure monitoring in patients with normal tension glaucoma (NTG)

Objectives: There are three objectives to this research. The first objective is to explore the pattern of intraocular pressure and blood pressure fluctuation in patients with presumed NTG. The second objective is to determine the proportion of patients with NTG but have increased intraocular pressure above 21mmHg when measured for 48 hours. Last objective is to determine the timing of peak and trough IOP and ocular perfusion pressure (OPP).

Introduction: Pathogenesis of glaucoma is not completely understood despite advancement in glaucoma research. One of the major risk factors of glaucoma is an increase in IOP but a high proportion of patients have glaucoma without raised IOP. One of the current major limitation in management of glaucoma is the infrequent number of IOP readings taken which are mainly within clinic hours only. Using home tonometry, patients are now able to measure their own IOP outside of clinic and thus opened up the possibility of studying IOP fluctuations.

Materials / method: 22 NTG patients from Princess Alexandra Eye Pavilion hospital were selected as participants. All participants had measurements of central corneal thickness (CCT), standard automated perimetry, baseline IOP and optical coherence tomography angiography (OCTA) imaging performed. Ocular medications were stopped for 4 weeks. They then had their corneal hysteresis measured and were fitted with Ambulatory Blood Pressure monitor and was instructed to take their own IOP at 4am, 9am, 11am, 1pm, 4pm, and 8pm for 2 days. Total of 13 IOP readings were taken and analysed together with blood pressure.

Results: The demographic and clinical characteristics of participants were separated into worse eye and better eye and tabulated. The results showed that 38.6% of patients had a peak IOP of >21mmHg during home monitoring despite IOP never exceeded 21mmHg during NTG follow up. It also showed that 9:00, 11:00 and 13:00 are the time when patients have the most peak IOP (70.1%) whereas 4:00 and 20:00 are more frequent for trough IOP (75.5%). Graphs illustrating home IOP measurements in worse eye, ocular perfusion pressure and mean arterial pressure for all participants were also drawn.

Conclusion: In conclusion, home monitoring is a feasible way to obtain 48 hours intraocular pressure. 38.6% of participants with NTG after washout period had a peak IOP of >21mmHg when monitored for 48 hours. This brings on the question of whether we should be using the term normal tension glaucoma based on a cut off IOP of 21mmHg. In addition to that, the data shows that peak IOP occurs more frequently at 9:00, 11:00 and 13:00 which tells us that IOP fluctuations also play a role in the diagnosis of NTG as we might not always be measuring patients’ peak IOP especially during clinic hours.

Iris melanocytoma and secondary glaucoma – a case report

Objectives: Iris melanocytic tumors include freckle, nevus, melanocytoma, Lisch nodule, and melanoma. Melanocytoma presents as a deeply pigmented benign tumor, a specific variant of melanocytic nevus with a distinctive clinical and histopathologic presentation. It was originally described on the optic nerve head and rarely arises in the iris or ciliary body of the eye. Iris melanocytoma (IM) typically presents at a young age and usually demonstrates minimal growth over many years. The objective of this study is to report a case of a rare clinical entity that needed a multidisciplinary ophthalmological approach.

Introduction: Iris melanocytoma is usually an asymptomatic, darkly pigmented, domed lesion with a slightly granular surface usually present in the iris root in the inferior quadrant. IM demonstrates a tendency toward spontaneous necrosis and pigment dispersion which can cause secondary glaucoma.

Materials / method: Case report of a 66 years-old woman, with antecedents of melanocytic iris nevus of the right eye (RE) diagnosed several years ago, who came to our service because of diminished visual acuity (VA) on that eye.

Results: The patient presented a VA of 0,5 on her RE. She had conjunctival hyperaemia and a melanocytic iris lesion of 3mm diameter with iridocorneal angle invasion, peripheric corneal adhesion and a sectorial cataract. The intraocular pressure (IOP) was 44 mmHg and the optic disc excavation 0,8. Anti-glaucomatous therapy was initiated, with good response. After an ultrasound biomicroscopy (UBM) suspicious of malignant transformation, excisional biopsy was made and histopathology revealed an iris melanocytoma with no signs of atypia. Cataract surgery was done. Trabeculectomy was needed to control IOP.

Conclusion: The natural course is variable. Most cases remain stable and require no intervention. A cautious observation of a lesion is the treatment of choice for most IMs. In cases of unusual clinical presentation, documented rapid tumor growth or secondary glaucoma, surgical treatment is indicated. Excisional biopsy is both a diagnostic and therapeutic measure, it is indicated when the tumor is confined to 3-4 clock hours and without seeding.

Idiopathic intracranial hypertension in children

Objectives: Le but de cette étude est d’exposer un cas d'hypertension intracrânienne idiopathique (HTICI) chez le grand enfant et de préciser les particularités cliniques et pronostiques de cette pathologie à cet âge.
INTRODUCTION :
L’HTICI est généralement observée chez la femme jeune et obèse. Chez l’enfant elle est considérée comme une pathologie rare.
OBSERVATION :
Un enfant de sexe masculin, âgé de 15 ans, sans antécédents pathologiques notables a été adressé pour un Fond d’Œil (FO) suite à des céphalées et des vomissements évoluant depuis quelques jours.
L’examen ophtalmologique au niveau des deux yeux a objectivé une acuité visuelle à 10/10, un segment antérieur normal, un tonus correct et un réflexe photomoteur afférent conservé.
Au FO, un œdème papillaire stade 2 bilatéral a été constaté. Le champ visuel de Goldmann était peu modifié avec simplement un élargissement de la tache aveugle.L’examen neurologique était normal ainsi que les explorations neuroradiologiques.
L’étude du liquide céphalorachidien (LCR) a trouvé une pression d’ouverture à 25 cm H2O avec une composition normale (chimie, cytologie et bactériologie).
Le patient a été mis sous inhibiteurs de l’anhydrase carbonique (IAC) par voie orale avec une bonne évolution.
DISCUSSION ET CONCLUSIONS :
Dans la population pédiatrique, l’HTICI est une maladie rare avec une prévalence rapportée de 0,1 à 0,9 cas pour 100 000 enfants.Il existe une incidence croissante de l’HTICI chez les adolescents (12–15ans) par rapport aux enfants plus jeunes (2–12ans), probablement liée à une tendance à l’obésité chez les prépubères.
Contrairement aux adultes où il existe une forte prédominance féminine, son incidence est identique chez les enfants prépubères de sexe masculin ou féminin.
Récemment, le diagnostic positif de l’HTICI repose sur les critères diagnostiques révisés de Friedman et al (2013). Contrairement aux critères modifiés de Dandy (2002), les nouveaux critères excluent l’utilisation de symptômes cliniques et, chez les patients sans œdème papillaire, s'appuient davantage sur les preuves radiologiques.
Le traitement médical actuel consiste principalement à prescrire des IAC. La durée des symptômes serait moindre et le pronostic visuel meilleur chez les enfants par rapport aux adultes.Comme il s’agit d’une pathologie susceptible d’évoluer vers la chronicité, une surveillance rapprochée de l’acuité visuelle et du champ visuel est indispensable afin de détecter précocement toute détérioration.

Posner-schlossman syndrome: clinical and evolutionary particularities

Objectives: Objectifs:
Notre but est de décrire, à travers un cas clinique , les caractéristiques cliniques ainsi que le pronostic du syndrome de Posner-Schlossman.

Introduction:
Le syndrome de Posner-Schlossman, dénommé également crise glaucomato-cyclitique, a été décrit pour la première fois en 1948.
Il s’intègre au sein des uvéites antérieures non associées à des manifestations autres qu’ophtalmiques, d’origine inflammatoire mais d’étiologie à ce jour inconnue.

Matériel / méthode:
à propos d’un cas

Résultat:
Il s’agit d’une patiente âgée de 47 ans qui consulte aux urgences pour flou visuel et douleur de l’œil droit évoluant depuis quelques heures.
L’interrogatoire révèle des antécédents d'épisodes similaires depuis quelques années.
L'examen clinique de l’œil droit a objectivé une acuité visuelle à 9/10, un œil blanc sans cercle perikeratique, une cornée claire, des précipités retro- cornéen (PRC) non granulomateux centraux, un micro tyndall de la chambre antérieure, un angle irido-cornéen ouvert, un iris normal, un tonus oculaire à 48 mmhg, un cristallin clair et un fond d’œil normal.
Devant la discordance entre le tonus oculaire très élevé, la douleur ressentie intense et l’inflammation minime du segment antérieur, le diagnostic du syndrome de Posner-Schlossmana été suspecté.

Discussion et conclusions:
Le syndrome de Posner-Schlossman est une affection rare. Il représente moins de 1 % de toutes les uvéites confondues.
Plusieurs auteurs le considèrent comme une réaction inflammatoire particulière secondaire à une réplication du CMV.
Son diagnostic est clinique. Il est caractérisé par la survenue de crises récidivantes unilatérales d’uvéite antérieure hypertensive aiguë, peu inflammatoire. L’atteinte est unilatérale dans la majorité des cas. Pendant la crise, la tension oculaire peut atteindre des valeurs de 40 à 50 mmHg, avec un angle irido-cornéen ouvert. Classiquement, on observe un PRC central de taille moyenne entouré de quelques PRC de petite taille. Au niveau de la chambre antérieure, il existe parfois un discret tyndall cellulaire, sans composante protéique et sans synéchies.
Les crises sont limitées dans le temps et se résolvent en général spontanément. Le traitement permet de raccourcir la durée de la crise.
Le syndrome de Posner-Schlossman peut se compliquer avec le temps d’un glaucome secondaire chronique à angle ouvert d’où une surveillance particulière doit être instaurée.
