Objectives: To recognize the urgency of evaluating patients with transient visual loss and other potential vascular causes of visual loss or diplopia.

Introduction: Transient vision loss or diplopia may be the first indication of an underlying vascular abnormality or occlusion that could lead to further systemic or neurologic complications.

Materials / method: Literature review and author experience.

Results: A substantial percentage of patients with vascular disorders will present with neuro-ophthalmic symptoms that will lead to identification of the underlying disorder and define the appropriate treatment strategy.

Conclusion: Early recognition of possible vascular disease causing eye symptoms can prevent permanent visual and/or neurologic morbidity.
