Objectives: 1. Recognize the presenting signs and symptoms of ischemic optic neuropathies.
2. Differentiate artertic and non-arteritic anterior ischemic optic neuropathy.
3. Initiate appropriate investigations for these entities.
4. Prescribe effective and evidence-based therapies for NAION and AAION.

Introduction: NAION is the most common neuro-ophthalmic cause of sudden, painless vision loss in patients over age 50. It has been associated with a number of systemic risk factors that may be addressed to reduce risk of subsequent events. AAION causes profound, usually irreversible vision loss in elderly patients and should be recognized and treated promptly to avoid fellow eye and systemic complications.

Materials / method: Review of literature and retrospective analysis of UCHealth patients with sudden vision loss.

Results: NAION may present with acute central or peripheral vision loss, while AAION generally manifests as severe central vision loss to counting fingers or worse. Systemic symptoms of giant cell arteritis may not be present. Vision may worsen in both conditions even after presentation.

Conclusion: Prompt diagnosis and management of NAION and AAION is required to address systemic risk factors and to protect vision in the fellow eye; with AAION, there are additional risks of systemic complications for which rapid treatment should be started.
