Managing the dislocated IOL: an algorithm

Objectives: To have a system in place for managing dislocated IOLs.

Introduction: At an average of 8.5 years after uneventful cataract surgery, 1-3% of the cases present with a dislocated IOL. Many treatment options have been proposed. This presentation will demonstrate a systematic approach that is used by the author in his management of these cases.

Materials / method: The author will present his approach to dislocated IOLs, both in the bag and out of the bag. Short videos will be used to demonstrate some of the techniques.

Results: The current algorithm employed by the author will be presented.

Conclusion: Successful surgical management of dislocated IOLs is possible using a variety of techniques, some of which have only recently been introduced.

Managing the dislocated IOL: an algorithm

Objectives: There are various reasons for the IOL to dislocate at different time points after cataract extraction. Uneven capsular contraction and fibrosis associated with anterior capsulotomy which is irregular and having diameter exceeding IOL optics can lead to IOL optic tilt and/or torque. That problem can be managed with subcapsular fibrotic tissue peeling. When IOL malposition is associated with weak zonules, suturing of the lens to the to the sclera and/or to the iris can provide stable and long-term fixation. Novel intrascleral haptic fixation techniques including G. Scharioth tunnels and flanged IOL haptics (S. Yamane technique) are also very reliable. These options are requiring the use of three-piece IOL lenses. IOL exchange with Iris-claw lenses provide similar results with scleral fixation but might be easier to accomplish for the anterior segment surgeon. This video-based presentation of a series of cases will provide tips for safe and efficient surgery.
