Objectives: PresbyLASIK treatments are currently designed regarding two distinct strategies: on the one hand depth of focus is enhanced through an adequate modification of the corneal asphericity, and on the other hand different power zones are created within the functionnal optical zone to permit vision at different distances.
We propose to compare those different strategies in application to the most popular presbyopic treatments available on excimer laser platforms.

Objectives: L'objectif est de savoir comment gérer les patients opérés de presbylasik ou de lasik hypermétropique au moment de la récidive de la presbytie en leur proposant les différentes options chirurgicales qui s'offrent à eux.

Introduction: La presbytie est un mécanisme physiologique évolutif. De ce fait, tant qu'aucun geste chirurgical radical sur le cristallin n'aura eu lieu, ce phénomène aura tendance avec le temps à récidiver. Les chirurgies réfractives cornéennes visant à compenser la presbytie n'auront qu'une vie limitée dans le temps et se posera tôt ou tard la question d'une retouche chirurgicale.

Materials / method: L'objectif est d'illustrer par différents cas pratiques la gestion de la retouche de cas plus ou moins simple de patients opérés de lasik hypermétropique ou de presbylasik. Ces cas illustreront des patients opérés par une retouche réfractive cornéenne ou par chirurgie du cristallin.

Results: Il s'agit ici de s'adapter au plus proche de ce que désire le patient sans faire des promesses démesurées. Le choix entre simple retouche réfractive cornéenne et chirurgie du cristallin clair dépendra de nombreux facteurs qu'il faudra aborder avec le patient. Le choix de l'implant dans une chirurgie du cristallin dépendra du profil photoablatif de la cornée observé à la topographie et conditionnera le moyen de compensation de la presbytie retenu.

Conclusion: La gestion des retouches chirurgicales des patients opérés de presbylasiks ou de lasiks hypermétropiques reste sans doute encore aujourd'hui un vrai challenge pour les chirurgiens réfractifs.

Objectives: Presbylasik: la consultation pré-opératoire en pratique

4. que dire aux patients demandeurs d'un presbyLASIK ?

Introduction: Le presbyLASIK est une technique efficace de traitement de la presbytie.
Cependant, elle nécessite une sélection rigoureuse des patients en pré-opératoire.
La conduite de la consultation pré-opératoire est ainsi cruciale. Le praticien doit se concentrer sur des élements cliniques, paracliniques et psychologiques afin de bien choisir le bon patient, gage de bon résultat final.

Materials / method: Expérience clinique

Results: A retenir:
1. protocole clair et bien expliqué au patient
2. simulation en lentilles indispensable
3. privilégier des patterns de traitement laser simples et réversibles
4. analyse précise de la topographie, notamment angle kappa et blanc-à-blanc

Conclusion: Le presbyLASIK est une approche efficiente pour la compensation de la presbytie par voie cornéenne.
Cependant, une évaluation pré-opératoire soigneuse lors de la consultation est essentielle dans la recherche d'un résultat optimal.
