Objectives: Moderate to severe subluxation of the natural crystalline lens can be effectively treated using the conventional and modified CTRs, capsular tension segment or capsular Anchor sutured to the scleral wall. My current approach to address the severe zonular deficiency is to use the capsular hooks together with Malyugin modified CTR sutured to the sclera with GoreTex. In some cases that is combined with femtosecond laser assisted anterior capsulotomy.

Objectives: To present tips for management of cataract in nanophthalmic eyes.

Introduction: Complications at cataract surgery in nanophthalmic eyes have been reported to occur in up to 30% of cases. The author will present his approach for minimizing these.

Materials / method: Video presentation of case of nanophthalmos and the approach used to limit complications.

Results: Good surgical outcomes are possible with nanophthalmos.

Conclusion: Nanophthalmos needs to be respected but not feared.
