Objectives: 1: Be able to identify the exact localisation of the haemorrhage
2: Be familiar with the different therapeutic modalities

Introduction: Massive submacular hemorrhage is a potentially devastating complication of certain retinal diseases, particularly neovascular age-related macular degeneration (nAMD) and retinal arterial macroaneurysms (RAMs). Although there is currently no “gold standard” for those haemorrhages, timely intervention is required.

Materials / method: Retrospective case series included 40 patients presented with submacular haemorrhage (SMH). All patients underwent an ophthalmological examination. The exact location of the hemorrhage was also analysed by SD-OCT. The common surgical procedures for all patients were pars plana vitrectomy with subretinal injection of tissue plasminogen activator (rtPA). The main outcomes measured were the best corrected visual acuity (BCVA) and anatomic parameters by SD-OCT: central macular volume (CMV), central macular thickness (CMT) and maximum macular thickness (MMT)) at the time of diagnosis and after surg

Results: The study included 26 patients with SMH secondary to nAMD and 14 due to RAM. In nAMD group, BCVA ranged from light perception (LP) to 20/200. In RAM group, BCVA was between HM and 20/160. The CMV, CMT and MMT were similar in the two groups. The mean for surgery was 6.0 days in nAMD and 4.3 days in RAM. In nAMD group, 35% of the patients had intravitreal injection of anti-VEGF at the end of the procedure. In RAM group, internal limiting membrane peeling was also performed. Conclusion: Patients with submacular hemorrhage secondary to RAM achieved on average a better final BCVA than patients with hemorrhage secondary to nAMD. A higher MMT and CMV at the time of diagnosis were related to a worse final BCVA.
