Objectives: Nous faisons une mise à jour sur la prise en charge diagnostique des paralysies de la 4e paire crânienne.

Introduction: Le diagnostic clinique de paralysie de l’OS repose sur un faisceau d’éléments de l’examen oculomoteur (les classiques 3 étapes de Parks) : (1) la mesure de la déviation en position primaire, (2) sa variation dans les regards latéraux (augmentation de l’hypertropie en adduction), et (3) lors de la manœuvre de Bielschowsky (augmentation de la déviation lors que la tête est penchée sur l’épaule du côté de la paralysie). On peut y ajouter la constatation d’une excyclotorsion (objective/subjective).

Materials / method: Nous discutons la place des différents signes orientant vers une paralysie congénitale ou acquise de l'OS et la place de l'imagerie.

Results: En cas de paralysie congénitale, aucune imagerie n’est recommandée. En cas de paralysie acquise typique, la présence de signes associés imposera la réalisation d’une IRM cérébrale et orbitaire. En cas de paralysie acquise isolée, la prescription d’une imagerie n’est pas bien codifiée, car la plupart des cas étant des paralysies présumées congénitales, et cette imagerie a un mauvais rendement diagnostic (la découverte d’une pathologie causale est exceptionnelle).

Conclusion: Les paralysies de l’oblique supérieur (OS) sont considérées comme la cause la plus fréquente de déviation oculomotrice verticale. Leur diagnostic étiologique a beaucoup bénéficié des apports de l'IRM, avec notamment la définition de l'entité de paralysie présumée congénitale, mais l'imagerie systématique n'est pas recommandée.
