Objectives: Lateral canthal procedures are often indicated to correct or prevent lower eyelid malposition. When determining an appropriate lateral canthal procedure, planning is essential and includes proper analysis and identification of any contributory anatomical factors. To facilitate análisis we propose basics points before any blepharoplasty procedure

Materials / method: Common designated abnormal preoperative findings included a negative vector lid (62 percent)margin eversion (12 percent), scleral show (21 percent), neutral or negative canthal tilt (49 percent and 18 percent, respectively), lateral canthus -to -orbital rim distance of more than 1 cm (11 percent) and Arcus Marginalis OUTIE 70% and INNIE.

Conclusion: Specific findings on the preoperative physical examination identify when simple or more complex lateral canthal procedures should be performed.

Objectives: Au travers d'un cas clinique l'auteur présente la technique de conjonctivo müllerectomie pour la prise en charge des ptosis minimes.

Introduction: La cure chirurgicale des ptosis modérés peut s'avérer difficile par les techniques classiques de remise en tension de l'aponévrose du releveur de par l'ajustage fin qu'elles impliquent. La resection conjonctivo müllerienne ( RCM) quand elle est possible offre une alternative intéressante ; Il n'y a en effet pas de réglage per opératoire, la technique est standardisée, reproductible, et sans cicatrice cutanée

Materials / method: L'auteur décrit les indications , la technique opératoire et les résultats.

Results: Une analyse rétrospective de 48 cas de RCM pour 40 patients est rapportée avec un suivi moyen de 22,5 mois. Le taux de succès dépasse les 80% avec très peu de complications.

Conclusion: La RCM apporte aux patients atteints de ptosis éligibles à cette technique une solution simple et généralement très satisfaisante.

Objectives: Define dark circle physiopathology
Manage dark circle
Avoid and treat the complications

Introduction: Dark circles around the eyes are unesthetic conditions that can occur as a consequence of ageing process or after inappropriate blepharoplasty.

These conditions leads to a skinny look : supratarsal depression widening, tear through deformity, prominent orbital rims, upper eyelid lenghthening and crease attenuation and rise, enophtalmos.

Results: Main ageing factors are intraorbital fat moving and depletion, orbital widening, ROOF and SOOF atrophy and descent, orbicularis retaining ligament lenghthening, orbicularis oculi hyperaction and orbital rim recession.
The management of dark circles consists in restoring peri orbital volumes thanks to preperiostic lipofilling or fillers injections.
Sometimes additional surgery can be done to improve the global appearance like midface lift in order to replace midface soft tissues and mask the tear through deformity or lower eyelid herniated fat transposition to restore homogenic midface look.

Conclusion: Dark circles are unesthetic conditions that should be improved if well managed.

Treatment should be individualised for each patient .

In all cases a good knowledge of anatomy and physiopathology is necessary to provide the best management.

Objectives: Rejuvenation of the lower eyelid is a particularly challenging area for plastic surgeons because of its unique and delicate anatomy.

Introduction: Most surgeons primarily address only aesthetic concerns of the lower eyelid, including excess skin and fat. However, when evaluating patients for eyelid rejuvenation, it is crucial that surgeons consider both aesthetics as well as function. All blepharoplasties induces some changes on Facial Expression

Materials / method: We review pertinent anatomy, morphologies, The adoption of lateral canthal support procedures into your surgical armamentarium can be a valuable means of preventing postoperative lower eyelid malposition.

Conclusion: The adoption of lateral canthal procedures into your surgical armamentarium can be a valuable means of preventing postoperative lower eyelid malposition. In addition to providing additional support, lateral canthal procedures can enhance overall aesthetics and facial expression in the majority of cosmetic blepharoplasty cases.
