Objectives: Jugular paraganglioma is a locally invasive, benign tumor, usually presenting with lower cranial nerve palsies. They rarely present with symptoms and signs associated with raised intracranial pressure.
Chronic headache with migraine type features; the importance of detailed history and thorough examination.

Introduction: A 19 year old female, with no significant past medical history presented with a six month history of right sided parietal headache associated with nausea, vomiting, photophobia and blurred vision. The headache improved after vomiting, was worse at night and no relief with regular over the counter analgesia.

Materials / method: Full neurological and ophthalmological examinations were performed. Automated visual field assessments, optical coherence tomography (OCT) of optic disc and macula and magnetic resonance imaging (MRI) imaging of brain was carried out.

Results: Visual acuity was 6/6 bilaterally unaided. There was no relevant pupillary afferent defect. Ishihara colour vision 17/17 right eye and 13/17 in left eye. Clinical examination of the optic discs revealed mild indistinct margins bilaterally. MRI of brain and skull base revealed multiple pathologies including a large right jugular paraganglioma and bilateral vestibular schwannomas.
With further questioning, relatives had commented on a deepening of her voice over recent weeks. She denied any hearing loss or tinnitus.

Conclusion: A rare cause of chronic papilledema requiring multidisciplinary team input from Otolaryngology, Neurology, Neurosurgery, Endocrinology and Ophthalmology. This patient is currently undergoing investigation for Neurofibromatosis type 2 as the underlying aetiology.

Objectives: Le but de notre étude est d'identifier les particularités épidémiologiques et les principaux facteurs de risque des kératites bactériennes du sujet âgé (60 ans ou plus) dans notre service d'ophtalmologie.

Introduction: Cause non négligeable de cécité dans le monde, les kératites bactériennes présentent des particularités chez le sujet âgé, liées à la baisse des défenses immunitaires, à la modification du microbiote oculaire, et à la présence de facteurs de risque aussi bien locaux que généraux.Les kératites infectieuses du sujet âgé sont de pronostic plus sombre que chez le sujet jeune, et l'évolution dramatique vers la perforation cornéenne a lieu dans 27% des cas tandis qu'elle survient dans 10% des cas chez le sujet jeune.

Materials / method: Il s'agit d'une étude rétrospective des patients âgés de 60 ans ou plus, hospitalisés au sein de notre service d'ophtalmologie entre les mois de janvier et octobre 2017 pour prise en charge d'une kératite bactérienne. Les caractéristiques épidémiologiques (facteurs de risque, délai de consultation) et évolutives ( durée d'hospitalisation, pronostic et complications) ont été soumis à une analyse statistique.

Results: 20 patients âgés de 60 ans ou plus ont été pris en charge d'une kératite bactérienne entre janvier et octobre 2017.La durée d'hospitalisation moyenne de ces patients était de 11,7 jours. Les principaux facteurs de risque étaient une blépharite (70%), un antécédent de chirurgie de cataracte (50%), une kératopathie (30%) , une sécheresse oculaire (45%), contexte traumatique (15%), maladie générale (20%). Le délai moyen de consultation était de 14,95 jours.L'évolution défavorable vers la perforation cornéenne était statistiquement corrélée au contexte post traumatique et à la gravité de la lésion

Conclusion: L'association de plusieurs facteurs de risque locaux comme généraux liés à un vieillissement de la surface oculaire, à la modification du microbiote local, font que les kératites bactériennes du sujet âgé sont de pronostic plus sombre que celles du sujet plus jeune, aboutissant dramatiquement à une perforation cornéenne et à la nécessité de recours à la chirurgie. La prévention et la gestion des facteurs de risque contribueraient à une meilleure prise en charge de cette entité pathologique potentiellement cécitante.

Objectives: Using OCTA to assess macular perfusion in cases of diabetic macular edema compared to normal eyes

Introduction: Diabetic macular edema is the commenest cause of dimminution of visual acuity in diabetic retinopathy. It can occure with any stage of diabetic retinopathy where there is macular thickening and edema due to disruption of blood retinal barrier. OCTA can show the reduced macular perfusion as diffuse or focal loss which is a non invasive technique

Materials / method: The study included 50 eyes that were divided into 2 groups
Group 1: 25 eyes with diffuse diabetic macular edema associated with ERM
Group 2: 25 normal eyes
Each eye had full ophthalmological examination in the form of best corrected visual acuity, IOP, fundus examination and OCTA
OCTA measured the percentage capillary perfusion and vessel density in superior inferior temporal and nasal juxtafoveal and parafoveal areas correlated to inner retinal layer thickness then compared to normal eyes
Results were statistically analyzed

Results: significant reduced macular capillary perfusion and reduced vessel density was detected by OCTA in eyes with diffuse ME and ERM compared to normal eyes

Conclusion: OCTA is a non invasive technique that helps to detect perfusion affection in different types of diabetic macular edema and can be a tool for early detection of retinal perfusion affection and as well can be used for follow up along the treatment course

Objectives: The aim of this systematic review is to evaluate the long-term (follow up for more than 12 months) recurrence rate of pterygium in all of the described techniques for pterygium surgery.

Introduction: Pterygium is a triangular fibrovascular tissue over the cornea. Its treatment involves surgical excision. A variety of techniques and adjuvants are described. Controversies in its recurrence rates are common since most of the articles show data within the first 12 months.

Materials / method: Database search was performed in Pubmed and SciELO with the keywords Pterygium; Primary Pterygium; Pterygium Surgery. Randomized clinical trials, retrospective studies, more than 40 eyes involved, follow-up of more than 12 months, description of the technique used, report of primary pterygium recurrence rate and articles in English were included. Were excluded systematic reviews, metanalysis, letter to editor or editorials, studies without recurrence rates and studies with incomplete or inconclusive data. Prisma Statement model was used. Two independent observers analyzed all articles.

Results: 2435 studies were found.43 studies with a total of 3841 eyes were eligible. Mean recurrence rate for bare sclera was 43.24%. Combined with mitomycin C was 12.89%. With bevacizumab, doxorubicin and beta therapy was 13.91%. Sutured conjunctival autologous transplantation had 7.68% and with mitomycin C, 2.75%. With fibrin glue was 4.30%, adding mitomycin C, 11.90%, and bevacizumab, 2.38%. Amniotic membrane sutured with no adjuvant, with triamcinolone and triamcinolone with mitomycin C had 23.08%, 10.53% and 9.52%, respectively. With fibrin glue had 13.89% and when added triamcinolone 9.38%.

Conclusion: The removal of the pterygium followed by bare sclera showed the highest recurrence rates. Autologous conjunctival transplantation with adjuvants such as bevacizumab or mitomycin C showed the lowest recurrence rates.
