Objectives: Acquérir les connaissances concernant le diagnostic et la prise en charge des adultes présentant une ésophorie décompensée ou un strabisme divergent intermittent

Introduction: L’ésophorie décompensée est une forme relativement fréquente mais peu connue d’ésotropie acquise, responsable d’une diplopie. Les strabismes divergents intermittents sont réputés pouvoir apparaître tout au long de la vie. Ceux rencontrés chez l’adulte sont soit présents depuis l’enfance et ont pu évoluer, soit ils sont apparus de façon plus récente (et sont souvent plus symptomatiques).

Materials / method: Au travers de cas cliniques nous décrivons comment faire le diagnostic d’ésophorie décompensée et éliminer les autres étiologies d’ésotropies acquises, comment faire le diagnostic de strabisme divergent intermittent chez l’adulte (diagnostic positif, classification) et orienter la prise en charge.

Results: Le diagnostic d’une ésophorie décompensée typique est clinique. Il faut savoir éliminer les autres diagnostics d’ésotropie acquise, ce qu’un examen clinique précis peut le plus souvent faire facilement. Le recours à l’imagerie cérébrale ne se justifie que dans les formes douteuses. Le traitement repose sur une prismation ou une chirurgie oculomotrice. Les strabismes divergents intermittents sont fréquemment rencontrés chez l’adulte. Conclusion: L’ésophorie décompensée est une forme spécifique d’ésotropie acquise, bénigne, peu décrite dans la littérature, dont les caractéristiques sont à connaître pour éviter les explorations inutiles. La prise en charge du strabisme divergent intermittent de l’adulte est variable d’un patient à un autre fonction de son retentissement et des souhaits du patient.

Objectives: Deux cas cliniques démonstratifs de la prise en charge
1/ d'un strabisme précoce
2/ d'un strabisme partiellement accommodatif
sont présentés

Introduction: Dans le STRABISME PRECOCE, il faut
Prévenir l’amblyopie monoculaire (le sensoriel avant le moteur)
L'amblyopie binoculaire demeure encore incurable
Dans l'Esotropie précoce, l'injection de Toxine botulique est le traitement actuel de référence, entre 9 et 24 mois . La Chirurgie « conventionnelle » est proposée après 3 ans
Dans l'Exotropie précoce, la Chirurgie précoce est proposée entre 1 et 2 ans, puis vers 4/6 ans si besoin

Conclusion: Les STRABISMES ACCOMMODATIFS sont
1/ de formes pures dans 2/3 des cas ; le traitement est réfractif
2/ de formes partielles dans 1/3 des cas ; traitement est réfractif et chirurgical
La distinction des formes partielles en CRN ou en CRA se fait après le traitement chirurgical et l’obtention d’une orthophorie ou d’une microtropie résiduelle ; c'est un diagnostic a posteriori
En cas de formes partielles, il faut proposer une intervention après un/deux ans de traitement médical (cycloplégie, lunettes et traitement amblyopie), avant 6 ans si possible, pour espérer obtenir une vision binoculaire
